Autumn Care of Forest City is nestled in the scenic foothills of the Blue Ridge Mountains in Rutherford County. The facility is conveniently located within a short drive from 4 major hospitals known for their outstanding heart, cancer, and trauma centers. Additionally, Rutherford Hospital, a provider of general health services is just minutes away.
Autumn Care of Forest City offers 100 skilled and rehabilitation beds and 25 rest home/assisted living beds. Outpatient therapy services were added for the community in 2003. We are proud to be known as the “Rehabilitation Specialist†in our area.

Discharge planning begins when the patient first arrives in the facility. Discharge objectives are discussed with the patient and/or family. Some patients may be discharged to home, while others decide they still need assistance and remain at the facility. Either way, we work to train patient, staff, and family members in compensatory techniques and equipment needs. We also communicate with other community agencies to ensure a continuum of care.
