LaSalle Healthcare Center accommodates 124 residents. We provide short-term rehabilitation including physical, occupational, and speech therapies as well as long-term care nursing services. We specialize in neurological disorders, complex diabetes treatment, amputation recovery, and pain management. We welcome short stays (respite care). Transportation services are available for physician appointments and dialysis treatments. We accept Medicare, Medicaid and most private insurance plans.

Our nursing center provides a full range of medical services to treat the residents who live with us and the patients who come to us for short-term, episodic medical or rehabilitative care.

The care we deliver includes daily nursing, pharmacy, dietary and social services for all residents and patients, as well as a range of specific services. We invite you to visit and see for yourself the face of quality care.

Our Services

Complex Wound Care

Designed for residents recovering from serious wound complications or wound related surgery, our Wound Care Program offers individualized wound care as well as comprehensive wound management. Some of our services may include: the development of a interdisciplinary care plan by a dedicated ostomy nurse that addresses nursing, dietary and rehabilitation treatment regimes; specialized bedding; and post-surgical recovery. Our goals are to focus on the health of our residents, to prevent wound-related complications, and to provide cost-effective access to quality wound care.

Long-Term Care

Our reputation rests on the care we provide, whether it is nursing, therapy, end-of-life care or social services. Long-term care may offer 24 hours a day care for those who require minimal assistance as well as those with multiple healthcare and rehabilitation needs. This is delivered through an interdisciplinary approach to your loved one's care by a team of licensed and certified medical care providers. We strive to meet the social, recreational and spiritual needs of residents with stimulating activity programs and a comfortable, home-like environment.

Rehabilitation Services

Our physical, occupational and speech/language therapists work with residents nationwide confronting a variety of conditions, from stroke to dysphasia to Parkinson’s disease, as well as conditions resulting from elective surgeries, multiple trauma or recovery from amputations. The rehabilitation team creates an individualized treatment program developed with the interdisciplinary team of nursing, social services, activities and dietary in consultation with your physicians. The team members’ objective is to play an integral role in helping you regain your health and reach your maximum functional ability.
