Rex Rehabilitation and Nursing Care Center is conveniently located off of US 1 on South Hughes Street. Turn on Perry Street off of Williams Street/Hwy 55 in Apex. We are on the corner of Perry Street and South Hughes.
At Rex Rehabilitation and Nursing Care Center of Apex both residents and patients will find the joys of a warm comfortable environment combined with the security of knowing that they are on the road back to good health and independence. Our facility is designed with residents in mind, to accommodate individuals with physical limitations. Rex Healthcare continues its tradition of providing high-quality care through Rex Rehabilitation and Nursing Care Center of Apex where residents and patients receive superior rehabilitation as well as excellent nursing care. Our caring and professional staff uses a team approach to work with our patients, residents and resident’s families, along with our physician’s to develop individualized treatment plans to insure that optimal goals are reached.
Short Term/ Post Acute Rehabilitation Services:
Our short-term, post acute rehabilitation program is designed to help patients get back on their feet and regain their independence by combining comprehensive exercise, physical and occupational therapy. This unit is equipped with 34 short-term beds and is specifically designed for those individuals that are discharged from the hospital who are not quite ready to return homes. The rehabilitation programs available are intended to provide patients with both education and assistance to allow for maximum independence. A team of rehabilitation specialists that includes your doctor, nurse, social worker, physical and occupational therapists and a speech therapist will design a treatment program to address your specific needs. Once you are ready to begin, your daily schedule will include exercises and activities of daily living. Our team will meet each week to evaluate your progress and make any necessary adjustments to your rehabilitation program. When it is time for you to return home we will assist you with all areas of this transition and work with you to help ensure you are in a comfortable and safe environment.
A fall, stroke, or brief illness can deprive us of our customary feeling of independence and security. It is important for patients to know that there is help and relief available at Rex Rehabilitation and Nursing Care Center of Apex. Our programs are designed to improve the physical condition of patients with limited functioning. Education is also available on exercises that you can do once discharged from our facility as well as techniques to help prevent future injuries. We also offer education to family and loved ones on the use of assistive equipment.
•Physical Therapy
With the help of Physical Therapists, patients work toward independence, returning to a fully functioning lifestyle as soon as possible. We offer comprehensive evaluation and treatment. Other problems treated include range of motion, flexibility, endurance issues, balance and gait training, strength and body mechanics.
•Occupational Therapy
Patients in need of occupational therapy work with trained occupational therapists to master activities of daily living from dressing and feeding to cooking, shopping and writing.
•Speech Therapy
Our speech therapists evaluate and treat patients with swallowing problems, cognition problems, language delay and voice articulation.
Extended Skilled Nursing Care
Rex Healthcare continues its tradition of providing high-quality care through Rex Rehabilitation and Nursing Care Center of Apex where residents receive superior extended nursing care in a homelike setting. Residents will feel right at home in our spacious suites, whether it’s a private or a companion. All suites are furnished with a bed, dresser, bedside table, wardrobe, sitting chair and are cable and telephone ready. Each suite also features a half bath.
For the health and well-being of our residents, staff and family members, a Registered Nurse is accessible seven days a week. These staff members are available to answer any questions family members may have regarding the care of their loved ones and assist staff wherever needed.
Our social workers will assist and support the social, emotional and spiritual needs of both patients and residents. They will also facilitate the discharge planning in order to ensure the safety and well being of each patient.
A registered dietitian oversees the menu and meal planning to ensure the three meals and several snacks provided each day are both appetizing and nutritious. We can also accommodate special therapeutic diets. All meals are prepared and served to residents in accordance with their specific dietary needs. Residents will enjoy the company of their friends while dining in our family style dining area.
Recreational Activities:
Our experienced staff coordinates a full monthly calendar of group and individual activities for residents. Bedside activities are also provided for residents with limited movement. Transportation is made available to residents who wish to participate in offsite programs and activities. A few of our many programs are:

Therapeutic Gardens
Residents and patients can safely exercise of relax and enjoy the beauty of nature in the outdoor gardens and courtyards adjacent to each wing. Garden areas feature flowering shrubs, trees and perennials with walking paths nestled in the landscaping that are smooth and wide enough to accommodate wheelchairs and walkers.
